We’re building a better model for integrated care that benefits patients, providers, and payers alike.
Our integrated model results in combined clinical excellence, operational efficiency, and data and analytics to provide comprehensive care for hundreds of thousands of patients living with kidney disease.
We support physicians in all areas by expanding existing patient panels to reach more individuals, enhancing care through ancillary services, and driving better outcomes while delivering greater value.
Our provider group is the heart of our business.
What began as a single practice in Arizona has grown into a collaborative network of 600+ aligned providers across 15 states who share ideas and best practices among leading nephrologists focused on advancing quality and care access.
Nephrologists and their teams improve patient outcomes and drive cost-of-care efficiency. To support their success, the Panoramic Health platform, underpinned by data and analytics, provides the strategic, operational, and financial infrastructure they need, all while preserving clinical autonomy.
For us, integration means more than just connection. It also includes embedded systems and shared goals that together create a seamless, holistic care ecosystem that unites practices, research, and ambulatory surgery centers (ASCs) to deliver better care.
Our Physician Executive Body (PEB) brings together physician leaders from each of our partner practices, ensuring those of every size and region have a voice in shaping our direction. We create space for meaningful collaboration by having physicians on various committees — driving alignment, open dialogue, and a culture of shared accountability among peers.
Working closely with nephrologists is key to lowering the total cost of care. Our data and analytics-backed integrated model makes it easier to identify at-risk patients early to close care gaps and optimize patient outcomes.
Our partner practices use shared clinical protocols and infrastructure to make it easier to scale high-quality care across the board. By aligning both clinical and financial goals, we give nephrologists the tools to better focus on patients, not paperwork. And with access to data from over one million patient records, we offer predictive insights and continuous quality improvements that really make a difference. Here’s proof:
The breadth and scale of our provider group creates unique opportunities that can be offered to practices of any size. This opens the doors for practices to offer ancillary services that enhance patient care and drive sustainable growth.
For example, our practice-embedded clinical research platform, Panoramic Science, gives patients access to cutting-edge therapies for cardiovascular renal metabolic (CVRM) diseases in a community-based setting.
Additionally, our joint ventures in Ambulatory Surgery Centers (ASCs) provide an alternative to costly hospital-based care — improving patient experiences and outcomes, reducing hospitalizations, and empowering physicians with greater ownership in care delivery.

Panoramic Health’s centralized infrastructure is deployed locally and combines the strength of a unified system with the flexibility to meet individual practice needs.
Our regional and local teams work closely with each practice to support daily operations and long-term success. We also provide a proven, scalable playbook for onboarding, operational improvement, and organic growth.
Partner practices stay true to their local identity while benefiting from a strong national network. This flexible model is built to support all types of practices — big or small, urban or rural.
Ultimately, our shared services model empowers providers to focus more on patient care and less on administrative tasks.
